POSITION SUMMARY:
The Emergency Department Case Manager will facilitate appropriate admissions, transfers, assessments and discharge with alternative placement as indicated for patients seeking care in our Emergency Department. The Case Management process will be accomplished through a collaborative multidisciplinary approach.

1. Current licensure in Texas as a Registered Nurse or Licensed Vocational Nurse or experience in Information Management
(Medical Records).
2. Experience in health care delivery systems with focus on Case Management or Performance Improvement preferred.
3. Bachelor’s degree in Nursing, Social Work, or related field preferred.
4. Professional, Unrestricted License in the current State of practice.
5. 3-5 years experience in hospital or managed care case management.
6. Previous experience as an ED case manager preferred.

A strictly controlled visitor and masking policy is required for patient safety.
